This is my analysis
From what I have read, Ilocanos only started being a majority in N.Luzon and asserting themselves after WWII and they adjust and partially assimilate to the people that they migrating in which they did not do after WWII(they used to speak Ibanag and Pangasinense prior to WWII as 2nd languages in Pangasinan and Region 2), I think in this scenario, the sooner Luzon will be more majorly Tagalog speaking than OTL since the nobility there prefered Tagalog over anything else(which they are either covert or overt in displaying even outside of the Tagalog speaking areas) and WWII destroyed their power there, I think those diaspora Ilocanos in regions 2-3 would shift to tagalog as their second language and would be gradually assimilated outside Ilocos, I think Luzon might recover and reestablish their identity and history sooner(It hasn't even now IOTL).
Prior to WWII Visayans and Mindanaoans have stronger nationalism, I think they might have a chance of reasserting their independence here since the independentist sentiments there would be strong since Zamboanga and Cebu had their own independent Nationalism.
I think it could turn into a Yugoslavia situation in this case but the three island groups being split.

So a balkanized Philippines? Imagine Luzon having several nations. While Visayas would be a united Republic. Meanwhile, Mindanao would be split between Muslims and Christians.

It would not be the case since Luzon only has one set of Nobility, so a more Tagalized Luzon is in cards here and I read that Luzon already had Tagalog as informal lingua franca/working language in Luzon sometime prior to the Americans coming in, WWII set back that development. Visayas would split into many republics, Cebu and the Federal Republic of Visayas are likely revived. I think Mindanao would be more Chavacano speaking as well.

I could see the citizens of VisMin be allowed to served in the Spanish Armed Forces similar to how Latin American citizens have that privilege. In OTL, only the Philippines and Puerto Rico are excluded from that privilege.

I could see the citizens of VisMin be allowed to served in the Spanish Armed Forces similar to how Latin American citizens have that privilege. In OTL, only the Philippines and Puerto Rico are excluded from that privilege. An independent Mindanao would also be possible and Mindanao would be the only Romance speaking country in Asia.
